Vita Green Charitable Foundation (‘the Foundation’)
The Mission
- To bring health, youth and joy to all; i.e. through taking a wide rage of affordable, effective premium Vita Green health products to prevent disease, improving well-being and enhancing people’s quality of lives, eventually to help people staying healthy, fit, and happy.
- To offer aid to individuals or needy groups or other non-profit-making organizations by means of providing Vita Green health products, and / or financial grant.
The Foundation
The Foundation is operated and funded by the Vita Green Group. All applications will initially be evaluated by a Selection Committee (‘the Committee’) consisting of a team of 5 professionals. Approval of the Committee evaluated applications and allocation of selected Vita Green products and/or financial grant will be made by the Foundation at its discretion.

Application requirements:
For individual application, applicant should have obtained professional, non-profit-making organization or social welfare group as his/her referee, and provide relevant documents for the Foundation’s consideration. For organizations, they can submit their application to the Foundation directly. The targeted beneficiaries are limited to Hong Kong residents for the time being.
Application Procedures:
1) By Individual Applicant:
Individual applicants (‘Applicant’) are required to submit following supporting documents to the Foundation:- Vita Green Charitable Foundation application form ‘App. Group Type 1’ (can be obtained from download: .PDF || .DOC or contact the Foundation by phone #2901-6088/ fax #8148-0086 / email: vgcf@vitagreen.com / post: The Vita Green Charitable Foundation, GPO Box 866, Hong Kong)
- Copy of the applicant’s valid Hong Kong Identification Card
- Copy of the applicant’s current contact address, and contact phone number
- Doctor’s certificate of the applicant’s current health condition
- Choice(s) of Vita Green product(s) recommended / expected to be received by the applicant
- Duration of Vita Green product(s) needed
- Income proof provided by the Applicant
- Proof of Referee’s Hong Kong recognized professional qualification (e.g. Annual practicing certificate of medical doctor, dentist, social worker, registered nurse, Chinese Medicine Practitioner, architect, engineer, solicitor, etc.)
> Definition of Referee: currently Hong Kong Registered Entities (e.g. registered health care practitioners, non-profit-making organizations, social welfare groups, etc) - Referee’s valid License number, and name of association
- Referee’s current contact address, and contact phone number
* Individual applicant will receive acknowledgement note from the Selection Committee upon receipt of a complete set of the required documents as aforementioned by the Selection Committee.
* Individual applicants are requested to inform the Selection Committee in writing during the initial application submission process for any application requiring urgent attention.
Otherwise, individual applicant will receive notification from the Foundation 3 months after receiving the acknowledgement note to confirm whether the application is successful. If the application is successful, the individual applicant will also be notified about the date and delivery details for the approved Vita Green products.
* All information received from Applicants and Referees will be kept strictly confidential and will not be disclosed to any third party.
* The Foundation reserves the final right to approve, decline, or withdraw any submitted, evaluated, or approved application at all times.
2) By Organizations (‘The Organization’):
Professionals, non-profit-making organizations, social welfare groups registered in Hong Kong are required to submit following documents to the Foundation directly.
* Supporting documents to be submitted by the Organizations about their needs of Vita Green products and/or grants for project(s) / events to be held:

- Most recent annual report
- Most recent audited report
- Memorandum and Articles of Association
- Valid Registration certificate showing non-profit making status
- Valid Registration number, Contact address and Contact number
- Biography of Principal(s)
- Proposed project(s) plan(s), and feasibility assessment(s)
- Choice of Vita Green product(s) and/or grants (for projects or events) applying
- Any other scholars and/or investigators involved in the application (if the application involves collaboration with another institution, some background information about the respective institution is required)
* The Organization will receive acknowledgement note from the Selection Committee upon receipt of a complete set of the required documents as aforementioned by the Selection Committee.
* Organizations are requested to inform the Selection Committee in writing during the initial application submission process for any application requiring urgent attention.
Otherwise, an Organization will receive notification from the Foundation in writing 3 months after receiving the acknowledgement note to confirm whether the application is successful. If the application is successful, the Organization will also be notified about the date and delivery details for the approved Vita Green products and/or the grants.
